Required Documents
After downloading the form fill it dully and attach following documents before submission.
Three attested copies of Secondary School Certificate i.e Matriculation or O-Level
Three attested copies of Higher Secondary School Certificate i.e FSc Pre Medical or A-Level
O and A Level candidates will also submit equivalence certificate issued by IBCC.
Three attested copies of Form B and if candidate’s age is above 18 then he/she will have to furnish copies of computerized national identity card.
Three attested copies of father’guardian’s national identity card
Six recent color passport size photograph of applicant. Write your name on the back of each photograph
If you want to apply on seats reserved for overseas Pakistani then attach the proof of your parent’s employment in abroad.
Send the hard copies of all above mentioned documents and application form to the admission office of BDMC within the due date. In case of submission of false or forged documents, strict legal action will be taken against the applicants and his all dues will also stand forfeited.
