Documents Required For Admission 2024 in Chand Bagh School Muridke

Following documents are required along with the dully filled admission form;
1. Mark Sheet of previous examination result
2. School leaving certificate and character certificate issued by the head of the last institute
3. Dully attested birth certificate and Form B issued by NADRA
4. A detailed questionnaire on medical history and necessary details
5. Complete medical reports from the listed pathological labs
6. Consent form by Parents’/Guardians
7. List of authorized visitors’ along with pictures duly signed by parent or guardian.
8. Two passport sized photographs of student.
