Punjab Healthcare Commission (PHC) has established a complaint cell to address the complaints of patients against healthcare service providers i.e clinics, hospitals etc. Patients can now file their complaints on prescribed form, which can be downloaded through this page. You may also file your complaint online through the link given below.

Written application on prescribed form
Attested copy of CNIC
Affidavit
Proof of correspondence with the health-care service provider
Record of treatment/documents and receipts

Punjab healthcare commission will investigate each and every complaint on merit. Kindly don’t file bogus complaints as in this case you may be fined up to 200000 rupees. Aggrieved health-care service provider can also file complaint in PHC. For filling online complaint you will have to register yourself on the official website of Punjab Healthcare Commission (PHC). You need to provide your personal details for creating your account like name, phone number, email address, mailing address and password. You may file complaint against following health-care service providers.

You can not file a complaint after 60 days of arising cause of action. Procedural details have been given in image given on this page.Appeal against the decision of PHC can be filed in the court of District and Session judge.Patients their net of kin, legal representatives and healthcare service providers can file complaint in PHC. Complaints without signs/thumb impressions,affidavit or proof will be rejected. Remember you should first inform the concerned authorities of health-care service provider before filling complaint in the PHC. No court fee or services of lawyer are required for filling complaint in PHC. Time barred, anonymous, pseudonymous and sub judice complaints will not be entertained. If you have not been treated properly, or your rights as a patient have been violated then don’t waste time and file your complaint right now.
