University of Health Sciences UHS Lahore First Convocation 2015 Registration
University of Health Sciences Lahore is going to organize its first convocation on Friday, 18th December,2015. Students who have completed their graduation till December 2013 can get themselves registered for participating in the convocation. Registration forms are available on the official website of University of Health Science Lahore. Download the registration form and and submit it in the office of in charge degree section UHS Lahore.

Eligible Graduates
Following graduates are eligible to participate in the convocation.
1-Students who have completed their PhD degree till December 2013.
2-Students who have completed post graduation including MS, MD, MDS and MPhil from the UHS and its affiliated colleges of the UHS till December 2013.
3-Best graduates who have completed graduation including M.B.B.S, B.D.S and B.Sc (Hons) or post graduation from UHS and its affiliated colleges. Such best graduates will be awarded gold medals. List of these best graduates has been uploaded on the UHS website . Registration is also mandatory for such graduates.
Documents Required for Registration
Following documents are required for registration
1-Attested copy of transcript/DMC
2-Attested copy of applicant’s CNIC
3-Two colored passport size recent photographs
4-Attested copy of guest’s national identity card along with recent colored photograph. Just one guest is allowed for best graduate gold medalists.
5-Registration fee for graduate is 1000 and favor each guest is 500 rupees. Attach the bank draft or pay order drawn in favor of treasurer UHS Lahore along with registration form.
Last date for registration is 3o November, 2015. No form will be received after the due date and incomplete forms will be rejected.
